Performance Appraisal

Manager Form

This form is designed to be completed by managers of student employees as part of the student employee appraisal process at DePaul University. The objectives of the appraisal process are: Increased Communication, Improved Performance, and Student Employee Development. 
 
The appraisal should be a two-way discussion and review of student employees' performance throughout the review period (6-month introductory or annual). Upon submission of these forms, a copy will be sent to the student employee and manager (for individual appraisals). The appraisal process should go as follows: 

Step 1: Inform the student you will be completing a performance appraisal and scheduling a meeting with them to discuss the performance appraisal. 

Step 2: (Optional) Instruct the student to complete their self-evaluation form first. 

Step 3: Once the student completes the self-evaluation, you as the manager will complete this form. 

Step 4: Meet with your student employee(s) to discuss the performance appraisal and optional self-evaluation.
